Report Forms / Reporting Information
Organizations awarded funding for an application submitted using the Internet Grant Application Manager (iGAM) may access report forms by going to the iGAM My Account page. Log in to iGAM by entering the same email address and password used to complete the initial application. Contact us at email@example.com or 816.627.3452 for technical support.
All other organizations awarded funding should refer to their award letter to determine the appropriate report form template to download below. Download report forms by clicking on the respective icon and saving the report form. Once completed, email the report to firstname.lastname@example.org.
