If you were previously awarded a scholarship for the current academic year, complete the following form and attach your most recent cumulative transcript to release your scholarship payment for the next term. You must continue to meet the scholarship’s criteria to be eligible for payment. If you have received multiple scholarships, fill out one form, and list them in the additional comments.

I hereby understand and accept the terms of this scholarship award, as specified in my award letter that was provided to me. Further, I understand the following:
1. My award is only for the current academic year, unless otherwise stated in my award letter;
2. I must meet the GPA requirement explained to me in my award letter in order to receive my next term’s payment;
3. I must provide my cumulative transcript after each session in order to receive my next term’s payment.
4. If my scholarship is payable in multiple installments, in most cases, I will be required to complete a full-time course load to receive the next session’s payment. Semester session students are required to complete at least 12 credit hours per semester and Trimester/Quarter session students are required to complete at least 9 hours per session;
5. If I attend a school with Trimester/Quarter sessions, my scholarship will be paid in three equal installments instead of two, in most cases; and
6. If I do not meet the requirements explained to me in my award letter, I forfeit any unpaid scholarship payments.
